Intervention Logbook
Terms of use
Last updated 23 October 2026
These terms apply when you use Intervention Logbook, whether you install it from an app store or use it on the web, and when you use its website, interventionlogbook.com. The app is made by Dr Azhar Ali (“I”, “me”). By using it, you agree to these terms. If you sign off cases as a supervisor, they apply to you too. The privacy policy explains how your information is handled.
Who can use it
The app is for healthcare professionals, including those in training, aged 18 or over.
What it’s for
Intervention Logbook helps you record the image-guided procedures you perform or take part in, for your training, supervision, appraisal and personal audit.
- It isn’t intended for diagnosing, treating or managing patients, and nothing in it is clinical advice.
- The procedure lists, supervision levels and targets organise your records. They don’t decide what you’re competent to do; your supervisors and training programme do.
- Counts, percentages and rates in the app, such as diagnostic yield and complication rates, are worked out only from what you enter. Check them before you rely on them, and don’t present them as audited figures.
- Recording a result is for your own learning and audit. It doesn’t replace your department’s process for checking and acting on results.
- It doesn’t replace your official training portfolio (such as the RCR e-portfolio), your appraisal records or your employer’s records.
No patient-identifiable information
You must not enter anything that could identify a patient, such as a name, date of birth, address, or an NHS, hospital or accession number. This applies everywhere in the app, including typed text such as lesion sites, histology results, complications, feedback and reflections.
The app refuses NHS numbers and asks you to check anything else that looks like a patient detail, but no check can catch everything. You’re responsible for what you type, and for following your employer’s information governance rules and your regulator’s guidance on confidentiality. If you enter patient details despite these terms, that is your responsibility, not mine.
Honest records
- Record only procedures you actually performed or took part in, at the level of supervision you actually had.
- Sign-offs and reviews must be entered by the supervisor in person, using their own PIN, and only for work they supervised. Never share a PIN or sign off for someone else.
- False entries could be treated as a probity matter by your employer, training programme or regulator.
- Whether a training programme, panel or appraiser accepts records from the app is up to them.
Your data and backups
Your logbook belongs to you. It’s stored only on your device, so I can’t see it, restore it or recover it.
- Back up regularly with Back up now, and keep your backup password safe.
- Lock your device with a passcode, because anyone who can open the app on it can read your logbook.
- If you use the web app, add it to your Home Screen. Browsers can delete stored data, for example when you clear your browsing data or use private browsing.
Free to use
The app is free. If I introduce paid features, I’ll tell you before they start and update these terms. Anything you buy through an app store follows that store’s terms on payments and refunds.
Your licence and my rights
I give you a personal, non-transferable licence to use the app to keep your own logbook and, if you’re a supervisor, to sign off cases on a trainee’s device.
The app and its design, including the Intervention Logbook logo and icon, belong to Dr Azhar Ali. You may not copy, sell, rent or reverse-engineer the app, except where the law allows. The open-source software it uses is listed under Settings › About › Open-source licences and stays under its own licences.
If you send me ideas or suggestions, I may use them to improve the app without owing you anything.
Apple App Store and Google Play
If you got the app from Apple’s App Store, Apple’s standard licence agreement for apps (its Licensed Application End User License Agreement) also applies, and these terms add to it. If you got it from Google Play, Google Play’s terms apply to the download.
These terms are between you and me. Apple and Google aren’t responsible for the app or its content, for supporting it, or for dealing with any claims about it.
Test versions
Early versions shared for testing, for example through TestFlight, may change without notice and may contain mistakes. Please report problems with Send feedback.
Changes and availability
I’ll keep improving the app, so features may change. I may also need to pause or stop the app or the website. Where I can, I’ll tell you in the app beforehand, so you have time to back up or export your logbook.
No guarantee
I work hard to keep the app accurate and reliable, but it’s provided “as is”. I can’t promise it will always be available or free of errors, so check your exports and summaries before relying on them, for example in your portfolio or at an appraisal.
Liability
Nothing in these terms limits my liability for death or personal injury caused by my negligence, for fraud, or for anything else the law doesn’t allow me to limit. Apart from that:
- I’m not responsible for losses that weren’t reasonably foreseeable when you started using the app.
- I’m not responsible for losing your logbook because a device is lost, broken or reset, browser data is cleared, or a backup or its password is missing.
- I’m not responsible for what you or a supervisor enters, or for decisions you or anyone else makes using the app.
- I’m not responsible for loss of income, business or opportunity, including effects on your training, appraisal or revalidation.
- I’m not responsible for problems caused by things I don’t control, such as your device, its operating system, other apps, or services run by others, including Apple, Google and Cloudflare.
- My total liability to you for anything else connected with the app or website is limited to £100.
If you use the app for your work and break these terms, for example by entering patient details, and that leads to a claim against me, you agree to cover my reasonable losses and costs from it.
If you use the app as a consumer, these terms don’t affect your legal rights.
Ending
You can stop using the app at any time. Deleting it deletes your logbook too, unless you’ve backed it up or exported it first.
Law
These terms are governed by the law of England and Wales, and you can bring a claim in the courts of England and Wales. If you live in Scotland or Northern Ireland, you can also bring a claim in your own courts.
Contact and changes
Questions or complaints: [email protected].
I may update these terms when the app or the law changes. The date at the top shows the latest version, and I’ll tell you in the app about important changes before they take effect. If a court decides that part of these terms can’t be enforced, the rest still applies.
